You’ll receive a secure link via email to complete new patient forms online before your appointment. If that’s not possible, plan to arrive 10–15 minutes early so our team can help you complete them on-site.
Bring the following items to help us serve you efficiently:
Valid photo ID
Insurance card (and any required referral)
List of current medications and allergies
Comfortable, loose clothing for easy movement (sneakers recommended)
Your initial visit typically lasts about 1 hour and includes:
A detailed medical history review and physical assessment
Guidance around your personal goals and limitations
Hands-on evaluation including range of motion, strength, and posture
Beginning your personalized treatment plan (e.g. manual therapy, exercises, or modalities)
You’ll discuss recommended session frequency and the estimated duration of your care plan.
After your evaluation, your therapist will discuss a treatment timeline and book your upcoming sessions at the desk. You’ll be given home exercises and education to support your recovery. We’ll also maintain open communication—your feedback helps us tailor your plan as you progress.
